How to Report a Hazard
What you need
Section titled “What you need”- Member or Admin access to SteadyOn
- The location, description, and rough risk level of the hazard
- Click Hazards in the sidebar
- Click Report Hazard
- Enter a clear Title that describes the hazard — for example,
Exposed electrical cable near server room door - Add a Description with enough detail for someone unfamiliar to understand the hazard
- Enter the Location where the hazard exists
- Set Hazard Type:
Permanent— the hazard will exist until actively removedTemporary— the hazard is short-term (e.g., during construction work)
- Complete the Risk Assessment:
- Set Likelihood — how often is harm likely to occur?
- Set Consequence — how severe would the harm be?
- SteadyOn calculates the Risk Level automatically
- Enter any Control Measures already in place
- Set a Review Date — when should this hazard be reassessed?
- Assign a Responsible Person who owns this hazard
- Click Save Hazard
Using the Guided Wizard instead
Section titled “Using the Guided Wizard instead”For a more guided experience — useful for workers who are less familiar with risk assessment — use the Guided Wizard:
- Click More ▾ in the Hazard Register header
- Click Guided Wizard
- Answer the questions in plain language
The wizard translates your answers into a full hazard record with a risk assessment.
Importing multiple hazards from CSV
Section titled “Importing multiple hazards from CSV”If you have an existing hazard register in a spreadsheet:
- Click More ▾ → Import CSV
- Download the CSV template
- Fill in your hazards (one per row) and save as CSV
- Upload the file — SteadyOn imports all valid rows and reports any errors
What happens next
Section titled “What happens next”- The hazard appears in your register with a BRAG status dot
- If you set a review date, SteadyOn will flag it amber/red as the date approaches
- You can raise corrective actions from the hazard detail page
