How to Report an Incident
What you need
Section titled “What you need”- Member or Admin access to SteadyOn
- Basic details of the incident: who, what, where, when
- Click Incidents in the sidebar
- Click Report Incident
- Enter a clear Title — for example,
Forklift near-miss in loading bay - Add a full Description of what happened
- Select the Type:
Incident— harm occurredNear Miss— harm could have occurred but didn’tNotifiable Event— a serious harm event that WorkSafe NZ must be notified about
- Enter the Location
- Set the Date and Time the incident occurred
- Optionally link a Related Hazard — this connects the incident to a known hazard in your register
- Enter Immediate Control Measures — what was done right away to reduce the risk?
- Enter Long-term Corrective Actions — what will be done to prevent recurrence?
- Click Add Photo / Video to attach evidence (optional; up to 5 files)
- Click Report Incident
For notifiable events
Section titled “For notifiable events”If the incident type is Notifiable Event, a warning will appear reminding you to notify WorkSafe NZ. SteadyOn records the incident and your notification, but you must submit the notification to WorkSafe NZ directly at worksafe.govt.nz/notifications.
Notifiable events include:
- Death of a person
- Notifiable injury or illness (broken bones, hospitalisation, amputation, etc.)
- Notifiable incident (unplanned or uncontrolled collapse of a structure, explosion, etc.)
After reporting
Section titled “After reporting”- The incident status starts as Reported
- Move it to Under Investigation when you start looking into the cause
- When investigation is complete and actions are raised, move to Closed
Public incident reporting
Section titled “Public incident reporting”You can share a public link with workers and visitors so they can report incidents without a SteadyOn account. Find this link at the top of the Incidents page.
